What's up everyone and welcome to another episode of the Epstein Chronicles. In this episode
0:35
we're going to continue talking about the death of Jeffrey Epstein.
0:40
And this time we're taking a look at the investigation report that was filed by the medical
0:46
examiner's office and this portion of the report was prepared by someone named Kathleen Ligio.
0:53
So, let's jump in and let's see what she had to say about the death of Jeffrey Epstein.
1:01
Case synopsis. The decedent is a 66 year old white male prison inmate who was found hung by
1:07
the neck within his cell and was later pronounced dead at the hospital. Despite recessive efforts,
1:14
the NOK does not object to an autopsy, PMH, information is pending on receipt of the correction
1:20
facility medical health services records. Subjective findings. As per Dr. Dejerre of the ED
1:29
at New York Presbyterian Lower Manhattan Hospital, EMS found the decedent to be in cardiac arrest
1:35
with the ligature markings of the neck following an unknown downtime at the Metropolitan Correctional
1:41
Center. The decedent was innovated and was given
1:45
in epinephrine times four and biocarbonate times two along with CPR. ACLS was continued
1:55
at the ED. The decedent was given an additional epinephrine times one and biocarbonate times one.
2:04
However, the decedent was in acetal by cardiac monitor and the cardiac stand still by ultrasound.
2:12
He was pronounced dead at 736 hours, so 736 in the morning.
2:17
Ligature markings of the neck were observed by Dr. Dejerre in the ED as well as by EMS.
2:24
However, no ligature material was observed by either EMS or Dr. Dejerre in the ED.
2:31
No bloods were drawn and no fentanyl was administered in the ED or by EMS.
2:37
Dr. Dejerre was instructed to submit to OCME, a complete clinical summary worksheet,
2:44
the EMS, ACR, and ED notes. These were received except the ACR.
2:51
An email was sent to evidence request with CCs to evidence pickups and OCME,
2:56
tour commanders requesting that the ACR be obtained.
3:00
The undersign was accompanied by Nicholas Schultz, Deputy Director of Forensic Investigation
3:08
and Operations for AMLI Scene Investigation, Special Agent Redacted.
3:14
We're also present at the scene conducting their investigation, as per Redacted.
3:19
At MCC, the decedent was detained at MCC since 766 2019, on charges of sex trafficking conspiracy
3:28
and sex trafficking of minors as inmate number 763 18054.
3:35
He was placed in a cell on 777 2019, but was moved for psychological evaluation
3:42
on 78 2019 through 710 2019. His psychologist was redacted. He was then moved to special housing
3:51
unit from 710 2019 to 723 2019. However, on 723 2019, the decedent attended suicide by hanging.
4:04
In the early morning hours, this date, the decedent cellmate, inmate Nicholas Tartaglioni,
4:09
was awakened by a sound and saw the decedent. He started banging on the cell door to
4:14
alert the corrections officers, redacted, responded at 127 hours, and observed the decedent
4:21
to be either seated or laying in a fetal position on the cell floor, with a loose,
4:26
homemade news around his neck made from torn orange bedsheet. At this time, the decedent was found
4:32
to have arrhythmia around two-thirds of the circumference of his neck, two inches wide,
4:38
with sparing of the back of the neck. There were also some friction marks,
4:44
the decedent was transferred to suicide watch from 723 2019 to 729 2019. He was transferred
4:53
to the shoe again on 729 2019, where he remained until the DOD. He was in cell number 206,
5:01
originally this last time in shoe, but was switched to cell 220 at an unknown date and time.
5:08
The decedent had a cellmate inmate Reyes, however on the morning of 89 2019 inmate Reyes,
5:15
went to court and was released through the court. No cellmate was present at the time of the incident,
5:21
leading to the decedent's death. Information was taken from redacted and redacted
5:26
regarding this incident. Regular rounds of the inmate cells are conducted by the corrections
5:31
officers every 30 minutes, but no longer than at 40 minute intervals. It's unknown when the
5:37
decedent was last seen alive pending interview of MCC personnel. On the morning of 810 2019,
5:45
at 633 hours during these regular rounds officers redacted, found the decedent hung by the neck
5:52
within a cell number 220. However, the location within the cell and the position of the decedent
5:58
are unknown pending interview of MCC personnel. Redacted removed the decedent from an unknown
6:05
fixed object and placed them on the cell floor. The officers started CPR. In the meantime,
6:10
redacted, entered the cell, redacted to gover performing CPR, and the decedent was taken to the
6:17
medical health services area. EMS arrived and took over care for the decedent there.
6:23
Scene investigation details. The scene was within the decedent cell,
6:28
number 220 on tier L of the shoe within the MCC. Of the scene photos, INV scene 003 depicts the
6:37
sealed cell door to 220. Photos 004 depicts a straight ahead view from the now open cell door.
6:45
Photos 005 represents the left side of the room. Beyond the right side wall of Photos 004,
6:53
the right side of the room can be viewed in Photos 009. There is bedding material strewn
6:59
in the middle of the floor, including a mattress, presumably from the top bunk, and orange blanket
7:04
and sheets. There was a CPAP machine on the floor with an electric cord reportedly provided
7:10
by the MCC staff at bedtime, 2200 hours, and plugged into an outlet to the right outside of the
7:17
cell door. A adjacent to the toilet and sink is a small table and stool on the right side of
7:23
the cell. There was a handwritten note, it was not a suicide note, but its content,
7:29
vented frustration about conditions and experiences at the facility.
7:35
There was a bunk bed, the mattress was removed, and personal items were kept on the top bunk,
7:40
among these were medications and supplements, including vitamin C, 500 milligrams.

8:25
which were both sealed closed, multivitamins, a seal container of milk magnesium,
8:31
was also sealed and docusate sodium 100 milligrams.
8:38
The bottom bunk had rumpled up orange bedding, there were strips of orange cloth,
8:43
presumably from bedding located in different spots in the room.
8:47
Included amongst these was what appeared to be a makeshift clothesline between the bunk bed,
8:53
post with a pair of orange underpants hung from it.
8:56
Measurements of the bunk bed are set as follows. The height from the floor to the top bunk
9:03
was 58 and a half inches, the length of the bed set was 79 and a half inches.
9:09
The width across the bed, photo 052, depicts the bottom bunk mattress, was 25 inches.
9:16
Photo 49 shows the heights of the rungs of the ladder to the top bunk.
9:22
Redacted of the medical health service arrived to the scene and presented the AED with defibrillator
9:28
pads, used on the decedents along with the ligature, which appears to be a strip piece of an orange bedsheet.
9:36
As per redacted, the AED revealed no shockable rhythm on the decedent,
9:41
the ligature was collected by the undersigned and entered into the evidence locker cage
9:46
at redacted in Manhattan. As per redacted, he was made aware by MCC staff that the decedent
9:52
cellmate Reyes left. It is believed that some of the items in the cell may have been his,
9:58
which were not removed. Additional information, as per redacted confirmed that the decedents
10:06
and okay as brother Mark Epstein redacted, was notified of the decedent's death.
10:12
The undersigned, called the brother, and discussed OCME policy and procedure with regard to an
10:18
ME case. He does not object to an autopsy, however, he is requested to have present private
10:24
independent physician, pathologist as a second opinion for the findings at the OCM autopsy.
10:31
Dr. Jason Graham has agreed to allow this as a courtesy to the family with the following
10:37
conditions. The physician must be a forensic pathologist, he, she, must arrive at OCME at 521st
10:44
Avenue in Manhattan before or by 9am on 811 2019. Mark Epstein was made aware of this and he
10:53
understood. He was instructed to call OCME with his choice of the private pathologist beforehand.
11:00
He was provided with the Manhattan OCME office address, telephone number, and the
11:05
decedents OCME case number. He expressed that following completion of the autopsy, he intends to
11:12
have the decedents body ship the Florida. The visual identification process was also discussed
11:17
with the brother. Note, the decedent cell at MCC is being held pending the preliminary autopsy
11:24
It's requested that the assigned ME discuss the preliminary autopsy findings with FBI special
11:32
agent redacted. Disposition ME case hospital with MLIC and investigation
11:39
and okay does not object autopsy and okay plans to retain an independent forensic pathologist
11:45
to observe the autopsy. The information provided above is true and correct to the best of my
11:51
knowledge and belief. Electronically signed by Kathleen Ligio on August 11th 2019.
