In this patient with cough, the most likely diagnosis is, well, I see a lesion in the mid to lower esophagus, which is extremely bright on the PET scan, and so you got to be thinking about malignancy. Foreign bodies would not have uptake on PET. Asophageitis can have some uptake, but not this extent. Lymphoma is not bulky enough on the CT scan. So what you're seeing here is a dilated esophagus with some wall infiltration, really positive PET scan, and this is a very good example of a esophageal cancer.
