释迦牟尼佛在舍卫国给孤独园时，城里有一位大富长者，家里的钱财多得数不清。长者花重金聘娶了一位名门贵族的漂亮女子为妻，每天音乐欢歌不断，日子过得很快乐。后来十月怀胎生下一个男孩。
这个男孩一生下来就与众不同，遍身长疮，因为非常疼痛，整天呻吟号哭，没有一刻安宁，于是，长者夫妇就给他取了一个名字，叫做“呻号”。随着年龄逐渐长大，呻号身上的恶疮不断溃烂，流脓流血，痛苦不堪。长者夫妇虽然把全国的名医统统请来医治，但还是没有好转的迹象。
